RELEASE AND AUTHORIZATION FOR USE OF NAME, PHOTO, LIKENESS AND IMAGE

 

I,

do hereby grant to the Tempe Union High Schools Education Foundation (the “Foundation”) and their officers, directors, and agents, including but not limited to their clients, licensees, purchasers, publishers, and volunteers, (collectively referred to as “Foundation Personnel”) the irrevocable right to use my name and any photograph, likeness and image of any kind and nature (collectively “the Photos”) for sale and reproduction in any medium, including without limitation books, magazines, television, cable television, web/internet sites, press releases, promotional materials and advertisements, when related in any way to the Foundation or the Tempe Union High School District.

 

I warrant that the right to create or use the Photos belongs to me and no one else and that this Release will not infringe, otherwise conflict, or interfere with any rights belonging to any other person, firm or entity. I will not enter the Photos in any contest or sell the Photos to any person, firm or entity.

 

I understand and agree that Foundation Personnel shall have full rights to the Photos in both color and black and white renditions. The Photos may, without my approval, be cropped, used as a basis for additional images, or otherwise changed by Foundation Personnel and may be used in conjunction with designs or additional Photos.

 

I am voluntarily making this Release and Authorization for Use of Name, Photo, Likeness and Image (“Release and Authorization”) of my own free will and that nothing has been promised to me in the form of payment, item or otherwise for entering into this Release and Authorization.
